Description of Course
Throughout the year, CMS will issue guidance documents to clarify the Medicare Part D regulations. The long-term care guidance document outlines ten service and performance criteria that a pharmacy must meet to be considered eligible for inclusion in a Prescription Drug Plan (PDP) network as a long-term care pharmacy.These services are also to be included in the dispensing fee paid to the long-term care pharmacy. This teleconference will highlight key issues for pharmacists and other health care professionals working in long-term care.

  2. Describe and identify the ten long-term care pharmacy performance and service criteria.
  3. Describe and identify the formulary provision in the long-term care guidance document.
  4. Describe and identify the provisions for a transition process for level of care changes in long-term care.

Thomas R. Clark is the Director of the Policy and Advocacy Department for the American Society of Consultant Pharmacists.  Prior to this Tom served as the Director of Professional Affairs for ASCP.  Before joining ASCP, he was a Vice President of Consulting for a major long-term care pharmacy company.  He has worked in hospital, ambulatory and long-term care pharmacy.  He is a graduate of the University of Illinois College of Pharmacy, and has a Master’s Degree in Health Care Administration from Mississippi College.
